About Your Visit

The parking garage is now open with plenty of available free for parking. The free valet parking is still available, if you pull up to the front of the Physician Offices II building.

Being on time for your appointment is very important

We take pride in providing each patient with a very detailed and comprehensive appointment. In order for us to do that we need patients to arrive on time (or even better early) especially if you are a new patient; if patients arrive 15 minutes late or later, you may be rescheduled.

NEW PATIENTS

Help us make your appointment run smoothly please prepare by bringing:

  • Health Insurance Card/Information
  • A list of all medications you are taking
  • A list of your questions /concerns
  • A copy of your XRAYS / CT scans (not just the reports -please get the actual films)
  • Copies of records from doctors related to your sinuses/nose/allergies/lungs.
  • Complete the “New Patient” paperwork below
